Governance Operations

Governance Operations™

Most organisations govern AI on paper. Governance Operations™ is how Trestano makes governance run day to day — a continuous cycle that detects what you use, assesses it, intervenes before it goes live, and recovers when it doesn't behave.

Why policies aren't enough

Writing an AI policy is the easy part. The gap between the policy and the practice is where compliance failures — and audit findings — actually happen.

A policy is a statement. Operations are evidence.

A policy says what should happen. Auditors, customers and insurers ask what did happen — which tool was approved, by whom, on what date. Only operations can answer that.

AI adoption doesn't wait for approval

New tools arrive through procurement, marketing or an engineer's laptop. Without an operational cycle, AI enters the business faster than governance can see it.

Risk is not a one-off exercise

A risk assessment describes AI as it was on the day it was written. Models are updated, vendors change terms, usage drifts — governance has to move with them.

Incidents are inevitable. Unmanaged incidents are not.

Every organisation using AI will eventually face an unexpected output, a data exposure or an automated decision that goes wrong. What separates a governed business is what happens in the next hour.

The operating cycle

Six stages, running continuously. Together they turn governance from a document into a discipline.

  1. Detect

Continuously identify the AI actually in use — new tools, new integrations, new use cases — before they become invisible infrastructure. Detection covers what procurement knows about and what it doesn't.

  2. Assess

Evaluate each system for risk, data exposure and decision impact. Assessment is repeatable and proportionate: a marketing chatbot and a hiring tool do not carry the same weight.

  3. Intervene

Act before deployment, not after — approvals, conditions, guardrails or a documented decision not to proceed. Intervention rights make clear who can stop or constrain a system, and when.

  4. Recover

When something goes wrong, contain it: suspend the system, limit the damage, notify who needs notifying. Recovery is rehearsed, not improvised at the moment of failure.

  5. Learn

Feed every incident, near-miss and review back into the governance record. Lessons change controls, ownership and boundaries — otherwise the same failure returns.

  6. Improve

Raise the standard over time: tighter controls, better evidence, broader coverage. Improvement is what turns compliance from a project into an operating capability.

Attributable Authority™ in operation

Governance Operations™ keeps the authority defined in your architecture current. Attributable Authority™ connects each AI action to the person or governance process that authorised it — including the delegated scope, execution boundaries and review or withdrawal conditions.

Across Detect, Assess, Intervene, Recover, Learn and Improve, that record provides a basis for reviewing permission, escalating changes and identifying who can suspend operation.

Governance at Execution™

Five controls that keep AI authority valid

Authority is checked, not assumed. Each control answers one question at the moment an AI system acts.

  • AVCAuthority Validity CheckIs the authority still valid?
  • EBCExecution Boundary CheckDoes the action fall within approved limits?
  • CDRConditional Delegation RulesDo current conditions allow execution?
  • RAMRuntime Authority MonitoringIs the system still behaving as authorised?
  • EMEExecution-Moment EvidenceCan you show what was authorised, and when?

What running governance looks like

You know governance operations are working when these four things are true — and provable.

  • Approvals before deployment

Nothing goes live without a recorded decision — who approved it, against what criteria, with what conditions.

  • Monitoring after deployment

Live systems stay under review: usage, drift, incidents and changes are watched, not assumed.

  • Evidence on demand

When a customer, auditor or regulator asks what you do about AI, you show records instead of assurances.

  • Accountable people, not just documents

Every system has a named owner. Every escalation has a route. Every decision has a signature behind it.

Where Governance Operations™ fits

Governance Operations™ is the third stage of the Trestano Methodology™ — it runs the framework designed in the Governance Architecture Model™, and it feeds the continuous assurance that certification readiness depends on.
